CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Glecaprevir, Pibrentasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldenseal interacts with.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with Mavyret (glecaprevir and pibrentasvir) and goldenseal. Goldenseal contains berberine and hydrastine, which can slow down the CYP3A4 enzyme in your gut and liver. Since glecaprevir is partly handled by CYP3A4, taking goldenseal alongside it could let more of the drug build up in your blood than expected.
For most people that mainly means a higher chance of nuisance side effects like headache, tiredness, or nausea. This is an active medication (not a prodrug), so a slowdown means more drug, not less. The evidence here is moderate and somewhat mixed, so I wouldn't panic, but since you're treating hepatitis C and want this course to go smoothly, please run goldenseal past your pharmacist or doctor before combining them.

P-GLYCOPROTEIN SUBSTRATES
2 of 2Glecaprevir, Pibrentasvir also falls under P-GLYCOPROTEIN SUBSTRATES.
There is conflicting evidence about the effect of goldenseal on P-gp. In vitro research suggests that berberine, a constituent of goldenseal, modestly inhibits P-gp efflux. Other evidence suggests that berberine induces P-gp. In healthy volunteers, goldenseal modestly increases peak levels of the P-gp substrate digoxin by about 14%. However, it does not seem to affect other pharmacokinetic parameters such as area under the curve (AUC). This suggests that goldenseal is not a potent inhibitor of P-gp-mediated drug efflux. Until more is known, goldenseal should be used cautiously with P-gp substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Both medicines in Mavyret (glecaprevir and pibrentasvir) ride on a transporter in your gut and liver called P-glycoprotein, or P-gp for short. Goldenseal may tinker with that transporter, though the science is mixed. Some lab work suggests it slightly slows P-gp, which could let a bit more of these drugs build up; other studies suggest the opposite. In real people, the effect on a similar P-gp drug (digoxin) was small, so goldenseal does not appear to be a strong actor here.
Still, since this is your hepatitis C treatment and you want it working at full strength, I'd play it safe. If more of the drug lingers, you might notice headache, tiredness, or nausea more than usual. Please check with your pharmacist or doctor before pairing goldenseal with Mavyret rather than deciding on your own.

What Is Glecaprevir and Pibrentasvir Used For?
Brand name: Mavyret
Mavyret treats acute or chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 in adults and children 3 years and older who have no cirrhosis or have compensated cirrhosis (mild liver scarring that is still functioning).
Mavyret is also approved for adults and children 3 years and older with genotype 1 HCV who were previously treated with either an NS5A inhibitor or an NS3/4A protease inhibitor (two earlier classes of hepatitis C drugs) — but not both at the same time.
