Ibuprofen, Acetaminophen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goldenseal interacts with.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

If you're reaching for Advil Dual Action (which combines ibuprofen and acetaminophen), here's the gist. The ibuprofen part is cleared by a liver enzyme called CYP2C9. Goldenseal, in test-tube studies, can mildly slow that same enzyme down. In theory, that could let ibuprofen linger a bit longer in your body, which might nudge up the chance of typical ibuprofen side effects like stomach upset, heartburn, or, with heavier use, kidney strain.

The important catch: this is only lab data, and it has never actually been shown to happen in people. So I wouldn't lose sleep over it. The acetaminophen piece isn't really affected here. Still, if you take goldenseal regularly, mention it to your pharmacist or doctor so they can keep an eye on things.

Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.

Here's the situation with Advil Dual Action (which pairs ibuprofen with acetaminophen) and goldenseal. Goldenseal contains berberine, and in lab dishes and animal studies berberine seems to make platelets a little less sticky, meaning blood may clot slightly slower. Ibuprofen on its own already has a mild blood-thinning effect and can irritate the stomach lining. So in theory, taking both together could nudge your bleeding risk up a bit, things like easier bruising, a nosebleed that lingers, or stomach upset.

I want to be honest though: this hasn't actually been seen in people, only in the lab, so it's more of a caution than a proven problem. If you take this combo regularly, or you're on any other blood thinner, just run it by your pharmacist or doctor first.

Here's the practical takeaway with Advil Dual Action, which combines ibuprofen and acetaminophen. The concern on file is that goldenseal might slow down a liver enzyme called CYP2E1. Acetaminophen is partly handled by that enzyme, and CYP2E1 is the pathway that turns a small portion of acetaminophen into a byproduct that can stress the liver. In theory, blocking it could shift how your body processes the drug, but honestly the direction here isn't clear-cut, and neither painkiller leans heavily on CYP2E1 for its main effect.

The most important point: this is based only on lab dish studies, not real people. It has never been shown to matter in humans, so I wouldn't lose sleep over an occasional dose. Still, since acetaminophen and the liver go hand in hand, run regular goldenseal use by your pharmacist first.
