Theoretically, goldenseal might increase the sedative effects of CNS depressants.
Goldenseal contains berberine. Animal research shows that berberine can have sedative effects. However, this effect has not been reported in humans.

Here's the short version: Dayvigo (lemborexant) is already a sleep medicine that quiets brain activity to help you nod off, so its main job is to make you drowsy. Goldenseal contains berberine, and in animal studies berberine showed some sedating effects of its own. The theory is that stacking the two could deepen that drowsiness a bit.

To be clear, this is a theoretical concern only. We have not actually seen berberine cause this in people. Still, if you ever pair them, just stay alert for extra grogginess, next-morning hangover feelings, dizziness, or trouble fully waking up. Before adding goldenseal to your routine, it's worth a quick chat with your pharmacist or doctor to be safe.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Lemborexant also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the gist for your sleep medicine. Your body clears lemborexant (Dayvigo) using a liver enzyme called CYP3A4. Goldenseal tends to slow that enzyme down, so the drug can clear more slowly and build up higher than expected in your system.

What that looks like in real life is an exaggerated version of what Dayvigo already does: feeling extra groggy or drowsy, lingering next-morning sleepiness, dizziness, or trouble feeling fully alert. Driving the next day could feel riskier. The evidence here is moderate, not ironclad, but it points in this direction. Please don't stop or change either one on your own. Just give your pharmacist or doctor a quick heads-up before pairing them so they can weigh in.
