Goldenseal + Pentoxifylline
Pentoxifylline brand name: Trental.
Goldenseal has a moderate interaction with Pentoxifylline. Theoretically, goldenseal might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.

ANTICOAGULANT/ANTIPLATELET DRUGS
Why this applies to Pentoxifylline
Pentoxifylline falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Goldenseal interacts with.
Goldenseal contains berberine. In vitro and animal research shows that berberine can inhibit platelet aggregation. However, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist if you're thinking about pairing goldenseal with pentoxifylline (Trental). Pentoxifylline already works by making your blood a little thinner and easier to flow, which is how it eases the cramping and aching in your legs or feet. Goldenseal contains berberine, and in lab dishes and animal studies berberine seems to make platelets (the cells that help blood clot) a bit less sticky too. Stack those two together and the theoretical worry is a touch more bleeding or bruising.
I want to be straight with you: this hasn't actually been seen in people, so it's a low-level, on-paper concern rather than something well documented. Still, watch for things like easy bruising, nosebleeds, or bleeding that's slow to stop. Before adding goldenseal, run it by your pharmacist or doctor first.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Pentoxifylline page
“Pentoxifylline (Trental) improves blood flow in people with poor leg circulation partly by making platelets less sticky, so treat this list as one to read carefully rather than skim. Of the 218 supplements on file, 202 can also slow clotting, which means adding them stacks bleeding risk on bleeding risk.”

About Pentoxifylline
What Is Pentoxifylline Used For?
Brand name: Trental
Pentoxifylline extended-release tablets treat intermittent claudication. This is cramping leg pain during walking that comes from chronic occlusive arterial disease (long-term blockage of arteries) in the limbs.
It can improve function and symptoms. It is not meant to replace more definitive treatment, such as surgical bypass or removal of arterial blockages, in peripheral vascular disease.
