Here's the simple version: Perampanel (Fycompa) already tends to make people sleepy, dizzy, or a bit unsteady on their feet. Goldenseal contains a compound called berberine, and in animal studies it seemed to have a calming, sedative effect of its own. So the worry is that putting the two together could stack that drowsiness and leave you feeling more groggy, foggy, or off-balance than usual.

I want to be honest about how soft this evidence is. This is a theoretical concern based on lab and animal work, and that sedating effect hasn't actually been confirmed in people. Still, since Perampanel can already affect your alertness, it's worth mentioning to your pharmacist or doctor before adding goldenseal, especially if you drive or notice extra tiredness.

Perampanel also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, goldenseal might increase serum levels of drugs metabolized by CYP2C9.
In vitro research shows that goldenseal root extract can modestly inhibit CYP2C9. This effect may be due to its alkaloid constituents, hydrastine and berberine. However, this effect has not been reported in humans.

Here's the gist: your body relies on liver enzymes to break down Perampanel, and one of those is called CYP2C9. In test tube studies, goldenseal (thanks to its berberine and hydrastine) seems to gently slow that enzyme down. If that happened in real people, a little more Perampanel could hang around in your system, which might nudge up side effects like dizziness, drowsiness, trouble with balance, or mood and irritability changes.

Now, the honest part: this is only based on lab research, not on actual patients, so it's more of a theoretical concern than a proven problem. Perampanel is not a prodrug, so the direction here is more drug, not less. Still, since seizure medicines deserve respect, I'd just give your pharmacist or doctor a heads-up before pairing the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Perampanel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Goldenseal might increase serum levels of drugs metabolized by CYP3A4.
Most clinical and in vitro research shows that goldenseal inhibits CYP3A4 enzyme activity and increases serum levels of CYP3A4 substrates, such as midazolam. However, in one small clinical study, goldenseal did not affect the levels of indinavir, a CYP3A4 substrate, in healthy volunteers. This is likely due to the fact that indinavir has a high oral bioavailability, making it an inadequate probe for CYP3A4 interactions and/or that it is primarily metabolized by hepatic CYP3A, while goldenseal has more potential to inhibit intestinal CYP3A enzyme activity. Both goldenseal extract and its isolated constituents berberine and hydrastine inhibit CYP3A, with hydrastine possibly having more inhibitory potential than berberine.

Here's the deal with these two. Your body relies on a liver enzyme called CYP3A4 to break down and clear perampanel (Fycompa). Goldenseal tends to slow that same enzyme down, so the medication can clear out more slowly and build up to higher levels in your blood than expected.

For perampanel, more in your system can mean stronger side effects. Think extra drowsiness, dizziness, trouble with balance or coordination, and sometimes irritability or mood changes. The evidence here is moderate, and goldenseal is an active herb, not a tiny food amount, so it's worth taking seriously. Perampanel isn't a prodrug, so this works in the direction of more drug effect. Please check with your pharmacist or doctor before combining them rather than changing anything yourself.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
