Saxagliptin, metformin XR falls under ANTIDIABETES DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypoglycemia when used with antidiabetes drugs.
Goldenseal contains berberine. Clinical research shows that berberine can lower blood glucose levels. However, this effect has not been reported with goldenseal.

Here's the simple version: your Kombiglyze (saxagliptin plus metformin XR) already works to bring your blood sugar down. Goldenseal contains berberine, and in studies berberine on its own can also nudge blood sugar lower. So in theory, stacking the two could push your sugar down a little too far, which we call hypoglycemia. That can show up as shakiness, sweating, dizziness, a fast heartbeat, or feeling confused or weak.

I want to be honest about the strength of this, though. It's theoretical, based mostly on berberine, and this drop hasn't actually been reported with goldenseal itself. So I wouldn't panic, but I would mention it to your pharmacist or doctor before combining them, and keep an eye on your readings.

Theoretically, goldenseal might reduce blood levels of metformin.
In vitro research shows that goldenseal extract decreases the bioavailability of metformin, likely by interfering with transport, intestinal permeability, or other processes involved in metformin absorption. It is unclear which, if any, of metformin's transporters are inhibited by goldenseal. Goldenseal does not appear to alter the clearance or half-life of metformin.

Your Kombiglyze combines two diabetes medicines: saxagliptin and metformin. The piece that goldenseal may bump into is the metformin half. In lab (test tube) studies, goldenseal seemed to lower how much metformin gets absorbed from the gut, possibly by blocking the transporters that carry it into the body. If less metformin gets in, it could work a little less well, and you might see your blood sugar creep higher than usual.

I want to be honest about how soft this evidence is. It comes from in vitro research, not real people, so we don't truly know if it matters day to day. Goldenseal shouldn't change saxagliptin much. If you'd like to use goldenseal, check in with your pharmacist or doctor first, and keep an eye on your blood sugar readings.
