Why this applies to Spironolactone

Spironolactone falls under ANTIHYPERTENSIVE DRUGS — the group Goldenseal interacts with.

Theoretically, goldenseal might increase the risk of hypotension when taken with antihypertensive drugs.
Goldenseal contains berberine. Animal research shows that berberine can have hypotensive effects. Also, an analysis of clinical research shows that taking berberine in combination with amlodipine can lower systolic and diastolic blood pressure when compared with amlodipine alone. However, this effect has not been reported with goldenseal.

Here's the gist: goldenseal contains berberine, and in animal studies (plus some human work with the blood pressure drug amlodipine) berberine seems to gently nudge blood pressure downward. Since you're taking spironolactone partly to lower blood pressure and clear extra fluid, stacking goldenseal on top could theoretically add to that effect and drop your pressure a little more than expected.

In real life, that might show up as feeling lightheaded, dizzy, or woozy when you stand up. I want to be honest though, this is a theoretical concern based on pharmacology, and it hasn't actually been reported with goldenseal itself. One extra thing worth a mention: spironolactone holds onto potassium, so always loop in your pharmacist or doctor before adding goldenseal.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
