Cobimetinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your cobimetinib (Cotellic). Your body relies on a liver enzyme called CYP3A4 to break cobimetinib down. The berberine in goldthread may slow that enzyme down a bit. If that happens, cobimetinib could clear out more slowly and build up to higher levels than expected, which can mean stronger side effects like diarrhea, nausea, skin rash and sensitivity to sunlight, fatigue, or changes in heart and liver lab tests.

I want to be honest about the evidence though. This is a theoretical concern based on how berberine behaves in lab studies, and it has not actually been reported in people. Still, since cobimetinib is a serious cancer medicine, I'd play it safe and check with your oncologist or pharmacist before adding goldthread.

What Is Cobimetinib Used For?

Brand name: Cotellic

Cobimetinib is approved for two uses in adults. First, it is used in combination with another drug called vemurafenib to treat melanoma (a type of skin cancer) that cannot be surgically removed or has spread to other parts of the body — but only when the tumor carries a specific genetic change called a BRAF V600E or V600K mutation. A lab test must confirm this mutation before treatment begins.

Second, cobimetinib can be used on its own (as a single agent) to treat adults with histiocytic neoplasms — a rare group of cancers involving immune cells called histiocytes.
