Isavuconazonium Sulfate (IV) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the short version for someone on Cresemba (isavuconazonium). Your body clears this antifungal partly through a liver enzyme called CYP3A4. Goldthread contains berberine, which in early lab work looks like it may slow that same enzyme down. If that happened in real life, your Cresemba could clear a little more slowly and build up, which might mean more of its usual side effects like nausea, headache, low potassium, or liver enzyme changes.

Now the honest part: this is theoretical. It's based on how berberine behaves in the lab, not on real reports in people, so I don't want you to panic. Still, since you're treating a serious fungal infection, please loop in your doctor or pharmacist before adding goldthread rather than starting it on your own.

What Is Isavuconazonium Used For?

Brand name: Cresemba

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
