Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the basic idea with goldthread and your lung cancer medicine, Lorbrena. Your body uses a liver enzyme called CYP3A4 to break lorlatinib down and clear it out. Goldthread contains berberine, which lab tests suggest may slow that same enzyme. If it does, lorlatinib could clear more slowly and build up in your blood, possibly making side effects stronger. With lorlatinib that might mean more swelling, fatigue, nausea, mood or memory changes, or shifts in cholesterol and blood sugar.

I want to be honest about the evidence though. This is theoretical, drawn from how berberine behaves in the lab, and it hasn't been confirmed in people. Still, since lorlatinib is a serious cancer drug, please check with your oncologist or pharmacist before adding goldthread.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
