Why this applies to Ziprasidone

Ziprasidone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for ziprasidone (Geodon): your liver uses an enzyme called CYP3A4 to help clear it from your body. The berberine in goldthread may slow that enzyme down, at least in lab studies. If it does, ziprasidone could clear a little more slowly and build up, which might mean more side effects like drowsiness, dizziness, or lightheadedness when you stand. One thing worth flagging is that ziprasidone can affect your heart's rhythm (QT prolongation), so I'd rather not see its levels nudged up unexpectedly.

The good news: this is theoretical, based on how berberine behaves in the lab, and hasn't been reported in real patients. Still, please check with your pharmacist or doctor before pairing them.

What Is Ziprasidone Used For?

Ziprasidone is prescribed for adults with schizophrenia and for adults experiencing manic or mixed episodes of bipolar I disorder. It can be used on its own for acute manic or mixed episodes, or added to lithium or valproate to help maintain stability in bipolar I disorder long-term.

An injectable form is also available for rapid control of agitation in people with schizophrenia who need quick relief. Prescribers are advised to weigh ziprasidone's potential effect on heart rhythm when choosing among treatment options.
