Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.

Theoretically, grape seed extract may increase the levels of CYP2D6 substrates.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.

Here's the gist for grape seed extract and doxepin. Your body relies on a liver enzyme called CYP2D6 to help break doxepin down. In lab dish studies, grape seed extract appeared to slow that enzyme. If that happened in real life, doxepin could clear out more slowly and build up a bit, which might mean more of its usual effects, like next-morning grogginess, dry mouth, constipation, or dizziness.

I want to be honest about how shaky this is, though. It's only been seen in test tubes, never in actual people, so it's more of a theoretical heads-up than a proven problem. Everyday grapes or a glass of juice almost certainly won't matter. If you're thinking about a concentrated grape seed supplement, just run it past your pharmacist or doctor first.
