Flutamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Grape interacts with.
A small pharmacokinetic study in healthy adults shows that ingestion of 200 mL of grape juice decreases phenacetin plasma levels. This is thought to be due to induction of CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for flutamide: your body relies on a liver enzyme called CYP1A2 to activate it into its main working form, hydroxyflutamide, which does most of the cancer-fighting work. There's a small, lower-quality study suggesting grape juice may speed up (induce) CYP1A2 a bit.
If grape products nudge that enzyme to work faster, in theory they could shift how much active drug is around. Because flutamide depends on this enzyme to become active, the effect isn't simple to predict, and honestly this evidence is thin and theoretical. Eating grapes as food is unlikely to matter much. Before adding a concentrated grape seed extract or drinking lots of grape juice, please check with your oncology team or pharmacist so your prostate cancer treatment stays on track.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Flutamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the gist with grape seed extract and flutamide. Your liver uses an enzyme called CYP3A4 to process flutamide, and concentrated grape seed extract might nudge that enzyme. The tricky part is that the research disagrees with itself: some lab tests suggest grape seed extract slows CYP3A4 down (which could let flutamide build up and raise the chance of side effects like nausea, diarrhea, or liver strain), while animal studies suggest the opposite, that it speeds the enzyme up (which could lower flutamide levels and make it work less well).
The honest answer is we don't know the direction, and this has not actually been seen in people, only in test tubes and animals. Eating grapes is not a worry. Before starting a grape seed supplement, though, run it by your oncologist or pharmacist first.

What Is Flutamide Used For?
Brand names: Eulexin, Euflex
Flutamide treats prostate cancer in men. Eulexin capsules are used with an LHRH-agonist (a type of hormone-lowering therapy) for locally confined Stage B2-C and Stage D2 metastatic (spread) prostate cancer.
For Stage B2-C disease, Eulexin and the goserelin acetate implant start eight weeks before radiation therapy and continue during it. For Stage D2 disease, Eulexin starts with the LHRH-agonist and continues until the cancer progresses.
