Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Grape interacts with.

It is unclear if grape seed extract inhibits or induces CYP3A4; research is conflicting.
In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Your body uses an enzyme called CYP3A4 to help break down riociguat (Adempas). Grape seed extract may bump into that same enzyme, but here's the honest part: the research pulls in two directions. Lab studies hint it could slow the enzyme (which could let riociguat build up, raising the chance of low blood pressure, dizziness, lightheadedness, or headache), while animal studies suggest it could speed it up (which could weaken the drug's effect on your lung pressure).

So far this hasn't actually shown up in people, and it's based only on lab and animal work. Eating grapes as food is very unlikely to matter. A concentrated grape seed supplement is a different story, so please check with your pharmacist or doctor before adding one.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
