Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with pairing Gravel Root and your phenobarbital. Gravel Root may contain pyrrolizidine alkaloids, plant compounds that can be hard on the liver. Phenobarbital is a strong inducer of a liver enzyme called CYP3A4, meaning it revs that enzyme up. In theory, a faster-running enzyme could convert more of those alkaloids into harmful, liver-damaging byproducts. So this isn't about phenobarbital working differently; it's about the herb potentially becoming more toxic to your liver while you're on it.

I'll be honest: this is based on lab and animal data only, not solid human studies, so it's more theoretical than proven. Still, given Gravel Root's liver risks and weak evidence of benefit, I'd skip it and check with your pharmacist or doctor first.
