Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Hemp Agrimony interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this pairing. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that may be hard on the liver. Those compounds get processed by a liver enzyme called CYP3A4. Rifabutin doesn't get turned into something harmful here, instead it speeds that enzyme up (it's an inducer). The worry is that a revved-up enzyme could convert more of the hemp agrimony's alkaloids into their toxic, liver-damaging form, so the concern is really about extra liver stress, not about your rifabutin working differently.
I'll be honest: this is a theoretical idea based only on lab and animal work, not real human cases. That said, hemp agrimony isn't something I'd recommend swallowing anyway because of its liver risk. Please check with your pharmacist or doctor before combining the two.

Rifabutin capsules are used to prevent disseminated MAC (Mycobacterium avium complex) disease — a serious bacterial infection that can spread throughout the body — in adults with advanced HIV infection.
It is not approved as a treatment for active tuberculosis (TB), and it should not be used alone for MAC prophylaxis in anyone who has active TB.
