Sotorasib falls under HEPATOTOXIC DRUGS — the group Hemp Agrimony interacts with.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA). Theoretically, using hemp agrimony with hepatotoxic drugs might have additive adverse effects on the liver. Some drugs that can adversely affect the liver include acetaminophen (Tylenol and others), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), fluconazole (Diflucan), itraconazole (Sporanox), erythromycin (Erythrocin, Ilosone, others), phenytoin (Dilantin), lovastatin (Mevacor), pravastatin (Pravachol), simvastatin (Zocor), and many others.

Here's the concern with pairing hemp agrimony and sotorasib (Lumakras). This isn't about one speeding up or slowing down the other. Instead, it's about your liver taking a double hit. Sotorasib already can stress the liver, and your care team likely checks your liver enzymes regularly while you're on it. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds known to be hard on the liver.

Put those together and you may add extra strain, which could show up as rising liver numbers, fatigue, nausea, belly pain, or yellowing of the skin or eyes. To be fair, this is a theoretical worry based on how these substances behave, not proven cases. Still, with a cancer medicine this important, I'd avoid hemp agrimony and check with your oncologist or pharmacist first.

Hemp agrimony contains hepatotoxic pyrrolizidine alkaloids (PA) which are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the twist with this pairing: the worry isn't about your cancer medicine getting weaker or stronger. It's about the herb itself. Hemp agrimony can contain pyrrolizidine alkaloids, natural compounds that are hard on the liver. Sotorasib speeds up an enzyme called CYP3A4, and that same enzyme can turn those alkaloids into even more toxic byproducts. So in theory, taking them together could raise the risk of liver damage.

I'll be honest, this concern is based only on lab work, not on real patients, so it's theoretical. But sotorasib can already stress the liver on its own, so I wouldn't stack another liver risk on top of it. Please skip internal hemp agrimony and check with your oncology pharmacist or doctor first.

Hemp agrimony is a traditional European herb that has been used for digestion, colds, and as a general tonic, but there is very little reliable human research to support these uses. More importantly, the plant can contain pyrrolizidine alkaloids that may harm the liver, so most experts do not recommend taking it internally. Talk with your pharmacist or doctor before using any product that contains hemp agrimony.

About Sotorasib

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
