Aspirin, Codeine Phosphate, Phenobarbital falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

This combination pill contains codeine, which is the piece that matters here. Codeine is a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can relieve pain. Lab studies suggest higenamine may block that very enzyme. If that happened in people, your body could make less morphine from the codeine, so you might get less pain relief than expected.

Keep in mind this is only test-tube evidence and has not been shown in humans, so the real-world risk is uncertain. Separately, higenamine itself can raise heart rate and blood pressure, which isn't ideal when stacked on other stimulants. Before mixing the two, check with your pharmacist or doctor.

The combination product you're asking about (Phenaphen) contains aspirin, which is the key piece here. Aspirin already thins the blood by keeping platelets from sticking together, which is why it can cause easy bruising, nosebleeds, or stomach irritation. On paper, higenamine seems to do something similar. In animal studies it slowed platelet clumping and made clots smaller, so stacking it on top of aspirin could, in theory, nudge your bleeding risk a little higher.

I want to be honest that this is a theoretical concern based mostly on lab and animal data, not solid human studies. Still, since higenamine itself can stress the heart and blood pressure, I'd loop in your pharmacist or doctor before combining the two rather than guessing.
