Here's the thing with this combo: your medication contains hydrocodone, an opioid pain reliever that already slows down the brain and can leave you feeling drowsy, foggy, or slow to react. Hops have a mild calming, sleepy effect of their own. Put them together and those sedating effects may simply stack up, so you could feel more tired, dizzy, or out of it than you'd expect.

I want to be honest about the evidence here, though. This is mostly theoretical, based on animal studies and how these compounds behave, not strong human data. The aspirin part isn't really involved in this interaction. Still, since both can cause drowsiness, it's worth running it past your pharmacist or doctor before pairing them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aspirin, Hydrocodone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

This combo product pairs aspirin with hydrocodone, a narcotic pain reliever. Hydrocodone is actually a prodrug of sorts, with your liver converting it (partly through CYP3A4 and CYP2D6) into more active forms. The theory here is that compounds in hops might gently speed up CYP3A4. If that happened, it could in theory shift how hydrocodone is broken down and slightly change pain relief or how much of certain byproducts form.

Honestly, I wouldn't lose sleep over this one. The evidence is weak (mostly lab and animal data), and a human study showed only about a 7.6% change in a similar drug, which is too small to matter. The bigger practical concern is that both hops and hydrocodone can cause drowsiness, so you may feel extra sleepy. Check with your pharmacist if you take them together.

Hops are most often used for sleep and mild anxiety, frequently combined with valerian, but the human evidence is limited and not very strong.
