Belladonna, Caffeine, Ergotamine, Pentobarbital falls under CNS DEPRESSANTS — the group Hops interacts with.
Some animal research shows that hops has sedative effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of this combination product that matters here is the pentobarbital, a barbiturate that calms the brain and can make you sleepy. Hops are used as a mild sleep and relaxation aid, and at least in animal studies they seem to have their own gentle sedating effect. Put the two together and the worry is simply that the calming effects could stack up, leaving you more drowsy, foggy, or unsteady than you expected.
This is a theoretical concern based on how the ingredients act, not from strong human studies, so it is not a reason to panic. Still, with a barbiturate on board, I would not start a hops supplement without checking with your pharmacist or doctor first, especially before driving or at bedtime.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

This combination medicine (Micomp PB) includes caffeine, which your body clears mainly through a liver enzyme called CYP1A2. In a test tube, some flavonoid compounds in hops slow that enzyme down, which in theory could let caffeine build up and add a little extra jitteriness, a racing heart, or trouble sleeping. The good news is that this is very weak, lab-only evidence.
When researchers actually gave a standardized hops extract to people and measured caffeine levels, nothing meaningful changed. So in real life this interaction is unlikely to cause problems. Just keep in mind hops can make you drowsy, while the caffeine in this product is a stimulant. If you take both regularly, mention it to your pharmacist or doctor so they can double-check it's right for you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill (Micomp PB) blends ergotamine and caffeine for migraines along with belladonna and pentobarbital. The ergotamine and caffeine pieces are cleared partly by a liver enzyme called CYP3A4. Some lab and animal work hints that compounds in hops could nudge that enzyme to work a little faster, which in theory might lower drug levels slightly.
The good news: a human study found this kind of change was tiny (under 8%) and almost certainly too small to matter. I wouldn't expect hops to weaken your migraine relief or change how the sedative effects feel. One thing to keep in mind: hops can cause drowsiness, and so can the pentobarbital and belladonna, so you may feel sleepier together. If you notice that, mention it to your pharmacist or doctor.
