Aleve PM pairs naproxen (a pain reliever) with diphenhydramine, the same sleep-inducing antihistamine found in many nighttime products. Diphenhydramine already makes most people drowsy, foggy, and a little uncoordinated. Hops are used as a mild relaxant, and adding them on top could deepen that sedation, leaving you more groggy than expected, especially the next morning.

To be fair, this caution is mostly theoretical and based on animal studies, so the real-world effect in people isn't well proven. Still, since both can calm the nervous system, it's reasonable to be careful. Don't drive or do anything risky until you know how the combo affects you, and check with your pharmacist or doctor before using them together regularly.

Hops extract does not seem to affect the metabolism of CYP1A2 substrates.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

Good news here: the lab worry about hops slowing down a liver enzyme called CYP1A2 did not hold up in people. In a real study, hops did not change blood levels of a test drug that uses that same pathway. So hops are very unlikely to make your Aleve PM (naproxen plus diphenhydramine) build up or hit harder through that route.

The thing I'd actually keep an eye on is simple overlap. Both diphenhydramine and hops can make you sleepy, so taking them together may leave you more drowsy or groggy than you expect. Go easy until you know how the combo affects you, skip driving, and check with your pharmacist if you take this often.
