Why this applies to Dronabinol

Dronabinol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the short version: your body uses an enzyme called CYP3A4 to clear dronabinol out of your system. Some lab and animal studies hint that hops might speed that enzyme up a little, which in theory could nudge dronabinol levels down and slightly weaken its effect on nausea or appetite. Dronabinol is the active drug itself, not a prodrug, so it isn't the kind of flip you'd worry about with codeine.

Honestly, though, the human data is reassuring. A study using hops alongside a similar CYP3A4 drug showed only about an 8% change, which is too small to notice. I wouldn't lose sleep over this one. One thing worth a mention: both hops and dronabinol can make you drowsy, so the combo may leave you extra sleepy. Just run it by your pharmacist or doctor first.

Hops are most often used for sleep and mild anxiety, frequently combined with valerian, but the human evidence is limited and not very strong. What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
