Theoretically, concomitant use of large amounts of hops might interfere with hormone replacement therapy due to competition for estrogen receptors.
In vitro research suggests that certain hops constituents can competitively bind to estrogen receptors. However, most hops extracts contain very small amounts of these constituents.

Here's the gist with Ortho-Prefest (estradiol and norgestimate). This is a hormone replacement product where the estradiol portion works by activating estrogen receptors in your body. Hops contain a plant compound called 8-prenylnaringenin that can latch onto those same receptors. The concern is that, in large amounts, hops might compete for that space and slightly blunt how well your estrogen therapy works.

I want to keep this in perspective. This worry comes only from lab and test-tube studies, not from people, and most hops products actually contain tiny amounts of that active compound. So an occasional hops tea or a beer is very unlikely to matter. If you're taking concentrated hops supplements regularly, just mention it to your pharmacist or doctor so they can keep an eye on your symptoms.

Estradiol, Norgestimate also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Hops extract does not seem to affect the metabolism of CYP1A2 substrates.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

If you're taking Ortho-Prefest (estradiol with norgestimate) and thinking about hops for sleep, here's the honest picture. In a test tube, certain hops flavonoids looked like they might slow down CYP1A2, one of the liver enzymes that helps process estrogen. On paper, that could mean a little more estrogen hanging around. But when researchers actually tested a standardized hops extract in postmenopausal women, it did not change how the body handled a CYP1A2 marker. So the real-world concern here is very low.

One extra note: hops contain a strong plant estrogen, so combining them with hormone therapy is worth a quick mention to your doctor or pharmacist, just so everyone's on the same page.
