Why this applies to Macitentan
Macitentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Opsumit (macitentan). Your body clears this medication using a liver enzyme called CYP3A4. Some lab and animal studies hint that compounds in hops might speed that enzyme up a little, which could in theory clear macitentan a bit faster and slightly soften how well it works for your pulmonary arterial hypertension. Macitentan is not a prodrug, so faster breakdown would mean less drug effect, not more.
The good news: the real-world evidence is very weak. In a human study, a hops extract changed a similar CYP3A4 drug by under 8 percent, which isn't enough to matter. I wouldn't lose sleep over this, but since PAH is serious, mention any hops supplement to your pharmacist or doctor before starting it.

What Is Macitentan Used For?
Brand name: Opsumit
Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.
In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
