Why this applies to Toremifene Citrate

Toremifene Citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the situation with hops and your Toremifene (Fareston). Toremifene is cleared from your body by a liver enzyme called CYP3A4. In lab and animal studies, certain hops compounds (lupulones) sped that enzyme up a bit. In theory, that could clear Toremifene a little faster and slightly weaken its cancer-fighting effect. Toremifene is the active drug itself, not a prodrug, so faster breakdown means less, not more.

The good news: a human study found this effect was tiny (only about an 8% change), so it's unlikely to matter in real life. There's also a separate reason to be cautious here, hops carry plant estrogens, and your treatment works by blocking estrogen. Please run hops past your oncology team before using it.
