Why this applies to Lumateperone

Lumateperone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for Caplyta (lumateperone): your liver clears it mainly through an enzyme called CYP3A4. Lab and animal studies hint that hops might speed that enzyme up a little, which could in theory clear lumateperone a bit faster and make it slightly less effective. Lumateperone is the active drug itself (not a prodrug), so faster clearance would mean weaker effect, not extra side effects.

The good news is that this concern is mostly theoretical. In an actual human study, hops only nudged a similar CYP3A4 drug down by about 8 percent, which isn't enough to matter for most people. I wouldn't lose sleep over it, but since lumateperone treats something as important as schizophrenia, it's worth a quick mention to your pharmacist or doctor, especially if you also feel extra drowsy.

Caplyta (lumateperone) is approved for adults with schizophrenia, depressive episodes associated with bipolar I or II disorder (bipolar depression), and as an add-on treatment for major depressive disorder (MDD).

For bipolar depression, Caplyta can be used on its own or alongside lithium or valproate. For MDD, Caplyta is used together with an antidepressant — not on its own as a standalone antidepressant.
