Horny Goat Weed and Aspirin, Caffeine, Codeine Phosphate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Horny Goat Weed interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 3 of those categories, so it can interact with Horny Goat Weed in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Aspirin, Caffeine, Codeine Phosphate

Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product carries three ingredients, but the piece that matters here is the caffeine. Your body relies on a liver enzyme called CYP1A2 to clear caffeine. In test-tube studies, horny goat weed appears to slow that enzyme down, which could let caffeine linger longer and pile up. If that happened, you might feel more jittery, have a racing heart, trouble sleeping, or an upset stomach.

I want to be straight with you: this is only seen in lab dishes, not in real people, so it is theoretical. The aspirin and codeine parts are not the concern here. Still, if you notice extra wired or restless feelings, mention it to your pharmacist or doctor before changing anything.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 3

Aspirin, Caffeine, Codeine Phosphate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, horny goat weed might increase the risk of bleeding.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to keep an eye on with this combo. Your medicine contains aspirin, which already thins the blood a bit by making platelets less sticky. In lab and animal studies, horny goat weed seems to do something similar, slowing how platelets clump together. Stack the two and, in theory, you could bruise more easily or bleed a little longer from a cut.

I want to be honest about the evidence though: this has only been seen in test tubes and animals, never reported in actual people, so it's more of a caution than a proven problem. Watch for unusual bruising, nosebleeds, or bleeding gums, and check with your pharmacist or doctor before pairing them, especially if you take aspirin daily.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This combo product mixes aspirin, caffeine, and codeine. The piece worth noting is the codeine. In lab dishes, horny goat weed appears to slow down a liver enzyme called CYP3A4, which is one of the routes your body uses to handle codeine. If that enzyme runs slower, more codeine could linger in your system, possibly nudging up its effects like drowsiness, dizziness, constipation, or slowed breathing. Caffeine could also clear a bit slower, leaving you jittery.

Here's the honest part: this is only seen in test-tube research, not in real people, so it's theoretical and likely mild for most. Still, before pairing them, it's worth a quick chat with your pharmacist or doctor.

“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”

Horny Goat Weed
  • Erectile dysfunction
  • Low libido
  • Fatigue and low energy
  • Bone health
  • Joint and menopausal symptoms
  • General 'vitality' or performance

Horny goat weed (Epimedium) is a traditional Chinese herb most often marketed for low libido and erectile problems, but solid human evidence for these uses is lacking. While short-term use seems generally tolerated by some people, quality and safety are not well studied, so talk to your pharmacist or doctor before trying it—especially if you have heart problems or take other medicines.
