Here's the short version: propranolol already works by relaxing and narrowing blood vessels and slowing the heart, which lowers blood pressure. In lab and animal studies, horny goat weed seems to nudge blood pressure down too. Stack those two together and, in theory, you could see a bigger drop than expected, with possible dizziness, lightheadedness, tiredness, or feeling faint, especially when standing up quickly.

I want to be honest about how soft this is, though. This effect has only shown up in test tube and animal research, never reported in people, so it's more of a caution than a proven problem. Since propranolol oral solution is often used in babies with hemangiomas, never start or change anything on your own. Run it past your pediatrician or pharmacist first.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist with this pairing. Your body uses a liver enzyme called CYP1A2 to help clear propranolol. In lab dish studies (not in people), horny goat weed slowed that enzyme down. If the same thing happened in the body, propranolol could build up a bit and act stronger than expected. For an infant being treated for a hemangioma, that might mean a slower heart rate, lower blood pressure, sleepiness, or low blood sugar.

I want to be honest: this is only a theoretical, test-tube finding, and it hasn't been seen in humans, so I wouldn't panic. Still, since this is a baby on a heart medicine, please don't add horny goat weed without first checking with your pediatrician or pharmacist.

What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
