Riluzole falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist: your body clears riluzole mainly through a liver enzyme called CYP1A2. In test-tube studies, horny goat weed seems to slow that same enzyme down. If that happened in real life, riluzole could build up in your system and hit harder than expected, which might mean more of its usual side effects like nausea, dizziness, tiredness, or changes in liver tests.

The honest part: this is only laboratory data, not something seen in actual people, so it's theoretical at this point. Riluzole is not a prodrug, so the concern is too much effect rather than too little. Since ALS treatment is important, I wouldn't change anything on your own. Just loop in your pharmacist or doctor before adding this herb.

What Is Riluzole Used For?

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
