Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the situation with Retevmo (selpercatinib). Your body relies on a liver enzyme called CYP3A4 to clear this cancer medicine out of your system. In test-tube studies, horny goat weed appeared to slow that same enzyme down. If that were to happen in real life, selpercatinib could build up higher than intended, which might mean more of its known side effects, things like high blood pressure, swelling, diarrhea, tiredness, liver enzyme changes, or changes on a heart tracing (QT prolongation).

The honest part: this is only a theoretical, lab-based concern and hasn't been shown in people. Still, because selpercatinib is a serious cancer drug with real risks, I wouldn't combine the two on your own. Please check with your oncology team or pharmacist first.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
