Theoretically, horny goat weed might increase the risk of bleeding.
In vitro research and animal research shows that horny goat weed can inhibit platelet aggregation and thrombus formation. This effect has not been reported in humans.

Here's the simple version: Brilinta (ticagrelor) already works to keep your platelets from clumping together, which is exactly what protects your stents and lowers your heart attack and stroke risk. The concern is that horny goat weed may do a little of the same thing. In lab dishes and animal studies, it slowed platelet clumping too. Stack the two and, in theory, you could nudge your bleeding risk a bit higher.

I want to be straight with you: this has not actually been seen in people, so it's a theoretical caution, not a proven problem. Still, with a blood thinner like this I'd play it safe. Watch for easy bruising, nosebleeds, or bleeding that won't stop, and check with your pharmacist or doctor before adding this supplement.

Ticagrelor also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the connection worth knowing about. Your body relies on an enzyme called CYP3A4 to clear ticagrelor (Brilinta) out of your system. In test-tube studies, horny goat weed seems to slow that enzyme down. If that happened in a real person, ticagrelor could build up a bit higher than expected. Since ticagrelor is a blood thinner that keeps platelets from clumping, more of it in your blood could mean a greater chance of bruising or bleeding, like nosebleeds, bleeding gums, or blood that's slow to stop.

I want to be honest about the evidence, though. This has only been seen in lab dishes, never in actual humans, so it's theoretical at this point. Still, because bleeding risk is serious, please check with your pharmacist or doctor before pairing these two.

About Ticagrelor

What Is Ticagrelor Used For?

Brand name: Brilinta

Ticagrelor tablets are used to lower the risk of cardiovascular death, heart attack (myocardial infarction), and stroke in adults who have had acute coronary syndrome (ACS) — a group of conditions including unstable angina and heart attack — or who have a history of heart attack. It also reduces the risk of stent thrombosis (a blood clot forming in a coronary stent) in people who have been stented for ACS treatment.

Ticagrelor tablets are also used in people with coronary artery disease (CAD) who are at high risk for a first heart attack or stroke, particularly those with type 2 diabetes. Additionally, Brilinta is approved to reduce the risk of stroke in adults with a mild acute ischemic stroke or a high-risk transient ischemic attack (TIA, or "mini-stroke").
