Theoretically, caffeine in Ilex guayusa might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Here's the practical side of this one. Cangrelor (Kengreal) is a powerful IV antiplatelet medicine given in the hospital, usually during a heart procedure to keep your platelets from clumping and forming clots. The caffeine in Ilex guayusa is thought to have a mild blood-thinning, anti-clumping effect of its own. Stacked together, the worry is a slightly higher chance of bleeding, things like bruising more easily or bleeding longer from a small cut.

The good news: this is purely theoretical, based on how caffeine behaves in lab settings, and it has never actually been reported in people. Since cangrelor is given by your medical team during a procedure, they're already watching you closely. Still, it's worth mentioning your guayusa habit to your doctor or pharmacist so it's on their radar.
