Ilex Guayusa and Mexiletine

Here is the interaction we found between these two, with the evidence behind it.

Minor

MEXILETINE (Mexitil)

Theoretically, mexiletine might increase the levels and adverse effects of caffeine.
Ilex guayusa contains caffeine. Mexiletine can decrease caffeine elimination by 50%.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here the supplement is the one that builds up, not your heart medicine. Guayusa is basically a caffeine source, like a strong cup of tea, and mexiletine slows down how fast your body clears caffeine (roughly cutting that clearance in half). So the same guayusa tea can hit you harder and linger longer while you are on mexiletine.

What that feels like is too much caffeine: a racing or pounding heart, jitteriness, trouble sleeping, or feeling wired. That is worth noticing since mexiletine is treating a heart rhythm problem. This is a theoretical, minor interaction, and small amounts may be fine, but please check with your pharmacist or doctor before making it a daily habit.

From our pharmacist’s overview on the Mexiletine page

“Mexiletine (Mexitil) steadies the heart's rhythm, and its benefit depends on holding a fairly precise blood level, so the 152 supplements on this list deserve real attention rather than a shrug. Only one entry, activated charcoal, is rated major, because it can soak up the drug in the gut and cut how much actually reaches your bloodstr…”

About Ilex Guayusa

What is Ilex Guayusa?

  • Energy and alertness
  • Mental focus
  • Antioxidant support
  • Appetite control
  • Athletic and physical performance

Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.

About Mexiletine

What Is Mexiletine Used For?

Brand name: Mexitil

Mexiletine hydrochloride capsules treat documented ventricular arrhythmias that your doctor judges to be life-threatening. One example is sustained ventricular tachycardia (a fast, dangerous heartbeat from the lower heart chambers).

It is generally not recommended for milder rhythm problems. It should also be avoided for asymptomatic ventricular premature contractions (extra heartbeats that cause no symptoms).
