Here's the simple version: Pemoline is a stimulant medication that already revs up your central nervous system. Ilex guayusa is basically a caffeine-containing tea, and caffeine is a stimulant too. So when you put the two together, you're essentially stacking one stimulant on top of another. That can add to the side effects you might already feel from Pemoline, things like a racing or pounding heart, jitteriness, trouble sleeping, anxiety, or feeling on edge.

To be honest, this is based mostly on expert reasoning rather than hard studies, so it isn't a dramatic danger. An occasional cup probably won't cause trouble, but regular or strong guayusa intake is more likely to matter. If you notice your heart racing or you can't sleep, check in with your pharmacist or doctor before changing anything.
