Iron and Carbidopa, Levodopa, Entacapone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

LEVODOPA

Why this applies to Carbidopa, Levodopa, Entacapone

Carbidopa, Levodopa, Entacapone falls under LEVODOPA — the group Iron interacts with.

Iron might decrease levodopa levels by reducing its absorption.
Advise patients to separate doses of levodopa and iron as much as possible. There is some evidence in healthy people that iron forms chelates with levodopa, reducing the amount of levodopa absorbed by around 50%. The clinical significance of this hasn't been determined.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical issue with Stalevo (which combines carbidopa, levodopa, and entacapone) and iron. When iron sits in your gut at the same time as levodopa, the two can bind together and form a clump your body can't absorb well. Studies in healthy people showed this can cut the amount of levodopa that gets in by roughly half. Since levodopa is the part doing the heavy lifting for your Parkinson's symptoms, less of it getting in could mean more stiffness, tremor, slowness, or 'off' periods returning.

The good news is this is easy to manage by spacing the doses apart. The real-world impact isn't fully nailed down, so don't stop either one. Just ask your pharmacist or doctor how many hours to separate your iron from your Stalevo.

About Iron

What is Iron?

  • Iron-deficiency anemia
  • Low iron stores during pregnancy
  • Fatigue from iron deficiency
  • Heavy menstrual blood loss
  • Restless legs syndrome (with low iron)

Iron is an essential mineral your body needs to make hemoglobin and carry oxygen in the blood. Supplements are mainly useful for treating or preventing iron deficiency and iron-deficiency anemia, ideally after a blood test confirms you are low. Because too much iron can be harmful, take it only when needed and under the guidance of a pharmacist or doctor.
