L-tryptophan and Mephenytoin

Here is the interaction we found between these two, with the evidence behind it.

Major

CNS DEPRESSANTS

Why this applies to Mephenytoin

Mephenytoin falls under CNS DEPRESSANTS — the group L-tryptophan interacts with.

Theoretically, concomitant use of L-tryptophan with CNS depressants might cause additive sedative effects.
Clinical research shows that L-tryptophan can cause fatigue and drowsiness.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: Mephenytoin (Mesantoin) is a seizure medicine that already tends to make people feel sleepy, dizzy, or mentally foggy. L-tryptophan can add its own drowsiness on top of that. Because tryptophan gets turned into serotonin and then melatonin, it can leave you feeling tired and sluggish. Stack that with Mephenytoin and the sedating effect can pile up, so you may feel more drowsy, less alert, or unsteady on your feet.

This concern is partly theoretical, but tryptophan really does cause fatigue in studies, so I wouldn't brush it off. Please don't start or stop anything on your own. Check with your pharmacist or doctor first, especially around driving or any tasks that need full focus.

From our pharmacist’s overview on the Mephenytoin page

“Anyone taking mephenytoin (Mesantoin) for seizures should read this page with genuine care rather than as background noise. The biggest cluster of interactions piles sedation on top of a drug that already slows the nervous system, and alcohol, whether beer or wine, sits at the serious end of that group.”
