Here's the thing to keep in mind: inhaled treprostinil (Tyvaso) works to open up and relax the blood vessels in your lungs, easing that high pressure and helping you breathe and move a little easier. Regular licorice, thanks to a compound called glycyrrhizin, can nudge your blood pressure up and make your body hold onto sodium and water while dropping potassium. So licorice may work against what your treprostinil is trying to do.

This is more of a theoretical concern, and it really depends on the dose. A little licorice flavor now and then is unlikely to matter, but concentrated licorice supplements or eating it regularly can add up. Deglycyrrhizinated (DGL) licorice doesn't carry this risk. Since you're being treated for pulmonary hypertension, please run any licorice product by your pharmacist or doctor first.

Treprostinil (Inh) also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, licorice might increase levels of drugs metabolized by CYP2C8.
In vitro, licorice extract from the species G. glabra and G. uralensis inhibits CYP2C8 isoenzymes. Theoretically, these species of licorice might increase levels of drugs metabolized by CYP2C8; however, this interaction has not yet been reported in humans.

Here's the gist for your inhaled treprostinil (Tyvaso). Your body clears this medicine partly through a liver enzyme called CYP2C8. In lab studies (test tube only), licorice appears to slow that enzyme down. If that happened in real life, treprostinil could build up a bit and you might notice more of its usual effects, like headache, flushing, low blood pressure or dizziness, throat irritation, or an upset stomach.

The honest part: this is only a theoretical concern based on lab data, and it has never actually been reported in people, so I don't want to alarm you. Still, regular licorice can raise blood pressure and drop potassium on its own, which matters when you're treating lung pressures. Please check with your pharmacist or doctor before adding a licorice supplement, and consider DGL if you just want the soothing throat benefit.

Licorice root is a traditional remedy used for sore throats, coughs, and digestive complaints, but solid human evidence is limited for most uses. Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. What Is Treprostinil Oral Inhalation Used For?

Brand name: Tyvaso

Treprostinil oral inhalation is used to treat two serious lung conditions that raise pressure in the blood vessels of the lungs. It helps improve how much physical activity you can do.

Tyvaso inhalation solution and Tyvaso DPI are both indicated to treat pulmonary arterial hypertension (PAH; WHO Group 1) and pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3). Clinical studies establishing effectiveness in PAH primarily included patients with moderate symptoms (NYHA Class III) whose PAH was idiopathic, heritable, or related to connective tissue disease. Studies in PH-ILD primarily included patients with idiopathic interstitial pneumonia, combined pulmonary fibrosis and emphysema, or connective tissue disease-related lung disease.
