Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Azathioprine
Azathioprine falls under IMMUNOSUPPRESSANTS — the group Lychee interacts with.
In vitro, flavonoids extracted from lychee show immunostimulant effects. So far, this effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking behind this one. Azathioprine works by calming down an overactive immune system, which is exactly what you want when you're protecting a transplanted kidney or settling down rheumatoid arthritis. In lab dishes, certain plant compounds (flavonoids) from lychee seem to do the opposite and gently rev the immune system up. On paper, that could tug against what azathioprine is trying to accomplish.
I want to be honest with you though: this is purely theoretical. It has only been seen in test tubes, never reported in actual people, and it's rated a minor concern. Eating lychee as a normal fruit is very unlikely to matter. I'd be more cautious with concentrated lychee extract pills. Either way, run any supplement past your pharmacist or transplant team first.

From our pharmacist’s overview on the Azathioprine page
“Azathioprine (Imuran) quiets an overactive immune system, which is why people take it after a transplant or for autoimmune disease, and it is also why I read this interaction list with real caution. Fully half of the 106 supplements on file, 53 of them, appear because they stimulate immunity and could blunt the very effect you depend on.”

About Lychee
What is Lychee?
- General nutrition and antioxidant support
- Digestive complaints (traditional use)
- Cough and sore throat (traditional use)
- Blood sugar support (preliminary)
- Weight and metabolic support (preliminary)
Lychee is a sweet tropical fruit that is nutritious and a source of vitamin C and antioxidant plant compounds. While the fruit is a healthy food for most people, supplement-strength extracts have only limited research behind specific health claims, and eating large amounts of unripe lychee on an empty stomach has been linked to dangerous low blood sugar in undernourished children. Talk to your pharmacist or doctor before using lychee extracts for any health condition.

Azathioprine Is Linked to Lower Levels of One Nutrient
Natural Medicines records Azathioprine as a medicine that can lower these. This is not an interaction with Lychee — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
