Cabazitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for someone getting cabazitaxel (Jetvana): this chemo drug is cleared from your body mainly by a liver enzyme called CYP3A4. In lab-dish studies, neem leaf extract seems to slow that same enzyme down. If that happened in a real person, cabazitaxel could clear more slowly and build up, which may mean stronger side effects like very low blood counts (raising infection and bleeding risk), diarrhea, fatigue, and nausea.

The honest part: this is theoretical, based only on test-tube work, and has never been reported in actual patients. Still, with a powerful cancer drug, I wouldn't gamble. Please loop in your oncologist or pharmacist before adding any neem product so they can keep you safe.
