Caffeine (OTC drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

Here's the gist: your body relies on an enzyme called CYP1A2 to clear caffeine out of your system. In lab dish studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, caffeine could hang around longer and hit a bit harder than usual. Think jitteriness, a racing heart, trouble sleeping, or feeling wired and anxious.

The big caveat: this has only been seen in test tubes, never actually reported in people. So it's more of a theoretical heads-up than a proven problem. Still, if you take neem regularly and notice your usual cup of coffee or caffeine pill feels stronger, that's worth mentioning. Chat with your pharmacist or doctor before combining the two if you have any concerns.

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist: in lab tests, neem leaf extract slowed down a liver enzyme called CYP3A4, which is one pathway your body uses to clear certain drugs. If neem blocks that enzyme, those drugs could build up a bit and hit harder. With caffeine, that might mean feeling more wired than usual, with jitters, a racing or pounding heart, trouble sleeping, or an upset stomach.

The honest part: this is theoretical right now. It comes from test-tube research, and it hasn't actually been seen in real people. Most of caffeine is also broken down by a different enzyme (CYP1A2), so the real-world effect may be small. Still, if you notice extra jitteriness while using neem, ease up on caffeine and check in with your pharmacist or doctor.
