Caffeine (prescription drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for caffeine: your liver relies on an enzyme called CYP1A2 to clear it from your system. In lab studies, neem leaf extract seems to slow that enzyme down. If that holds true in people, caffeine could hang around longer and hit a little harder than usual. You might notice more of the typical caffeine effects, things like jitteriness, a racing or pounding heart, trouble sleeping, or feeling on edge.
I'll be honest with you though, this is only based on test-tube research, not real cases in humans, so the actual risk is probably small. Still, if you're taking prescription caffeine and want to try neem, it's worth a quick chat with your pharmacist or doctor first.

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: lab studies suggest neem leaf extract may slow down a liver enzyme called CYP3A4, which helps your body clear certain medications. If that enzyme is blocked, a drug it processes can build up and hit a little harder than expected. With prescription caffeine, more buildup could mean stronger stimulant effects like jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired and anxious.
I want to be honest with you though: this is purely theoretical from test-tube research, and it hasn't actually been reported in people. So I wouldn't lose sleep over it. Most of caffeine's breakdown actually happens through a different enzyme. Still, if you plan to use concentrated neem supplements regularly, run it by your pharmacist or doctor first.

What Is Caffeine Used For?
Brand names: Dextrophin, Caffeine
Caffeine citrate injection and oral solution are used for the short-term treatment of apnea of prematurity. This means pauses in breathing in premature infants. The label lists the age range as infants between 28 weeks and a later gestational age, and that range is cut off in the text provided.
Over-the-counter caffeine tablets, such as Diurex and the Stay Awake products, are taken by mouth by adults and older children. Their labels point to fatigue and drowsiness. Diurex directions also mention taking it while symptoms persist.
