Carbetapentane, Chlorpheniramine, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your Ricotuss combines a cough suppressant (carbetapentane), an antihistamine (chlorpheniramine), and a decongestant (phenylephrine). All three lean on a liver enzyme called CYP3A4 to be broken down. Neem leaf extract has been shown in lab dish studies only to slow that enzyme down. In theory, that could let these medicines build up a little, nudging up their usual effects like drowsiness, dry mouth, dizziness, a faster or pounding heartbeat, or a jittery, restless feeling.
Here's the honest part: this has never actually been reported in real people, only in test tubes, so I wouldn't lose sleep over it. Still, if you take neem regularly and notice extra grogginess or a racing heart, mention it to your pharmacist or doctor before changing anything.
