Neem + Cyproterone Acetate, Ethinyl Estradiol
Cyproterone Acetate, Ethinyl Estradiol brand names: Diane-35, Dianette.
Cyproterone Acetate, Ethinyl Estradiol falls into 3 drug categories that Neem can interact with; the most serious is rated moderate. Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Neem interacts with whole categories of medication. Cyproterone Acetate, Ethinyl Estradiol falls into 3 of those categories, so it can interact with Neem in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Cyproterone Acetate, Ethinyl Estradiol
Cyproterone Acetate, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with Diane-35 (cyproterone acetate plus ethinyl estradiol) and neem. Your body uses an enzyme called CYP1A2 to help clear estrogen out of your system. In lab dish studies, neem leaf extract slowed that enzyme down. If that held true in real life, your estrogen could be cleared a bit more slowly and build up slightly, which might mean more of the usual estrogen-type side effects like nausea, breast tenderness, headaches, bloating, or mood changes.
I want to be honest about the strength here: this comes only from test-tube research, and nothing like it has actually been reported in people taking this pill. So it's more of a theoretical caution than a proven problem. Still, if you're thinking about regular neem supplements, it's worth a quick chat with your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES
2 of 3Cyproterone Acetate, Ethinyl Estradiol also falls under P-GLYCOPROTEIN SUBSTRATES.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with neem and your Diane-35 (cyproterone acetate plus ethinyl estradiol). Neem leaf extract may block a transporter in your body called P-glycoprotein, which is one of the pumps that helps move drugs around and out. If that pump slows down, a little more of your medicine could stay in your system, which in theory could mean stronger effects or more of the usual side effects like nausea, breast tenderness, headaches, or mood changes.
I want to be honest about how soft this is. The concern comes only from lab studies, not from real people taking neem and birth control together, so it's more of a 'good to know' than a red flag. Still, since this combo also protects against pregnancy, run it by your pharmacist or doctor before adding neem supplements.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Cyproterone Acetate, Ethinyl Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining neem with Diane-35 (cyproterone acetate plus ethinyl estradiol). Both hormones in this pill are processed by a liver enzyme called CYP3A4. In test-tube studies, neem leaf extract seems to slow that enzyme down. If that happened in your body, the hormones could clear a little more slowly and build up, which might mean more of the usual side effects like nausea, breast tenderness, headaches, mood changes, or bloating.
The important thing to know is that this is theoretical. It comes from lab work, not from real people, so we honestly don't know if it matters in everyday life. I wouldn't lose sleep over it, but it's worth a quick chat with your pharmacist or doctor before starting concentrated neem supplements.

From our pharmacist’s overview on the Cyproterone Acetate, Ethinyl Estradiol page
“Diane-35 (cyproterone acetate with ethinyl estradiol) is a hormone pill, often used for birth control and acne, so a supplement that weakens it can mean breakthrough bleeding or an unplanned pregnancy. That is why the 211 entries here warrant real care, and the three major ones come first.”

About Neem
What is Neem?
- Skin conditions
- Dental and gum health
- Antimicrobial and antiparasitic
- Digestive upset
- Blood sugar support
- Insect repellent/pesticide
Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.
