Why this applies to Finasteride
Finasteride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP3A4 to help clear finasteride, and lab studies suggest neem leaf extract may slow that enzyme down. Finasteride is an active drug, not a prodrug, so if it clears more slowly, a bit more could linger in your system. In theory that might nudge up its usual effects, like reduced sex drive, trouble with erections, or breast tenderness.
The big caveat: this is only seen in test-tube and animal studies, and it has not been reported in real people. So I wouldn't lose sleep over it. Still, since neem itself can be risky if swallowed, please check with your pharmacist or doctor before pairing the two.

Neem is a tree from India used for centuries in traditional medicine, especially for skin, dental, and antimicrobial purposes. Some small studies are promising for oral health and skin, but high-quality human evidence is limited for most uses. Neem oil and seed extracts can be toxic if swallowed, especially in children, so use with caution and talk to your pharmacist or doctor first.

What Is Finasteride Used For?
Brand names: Propecia, Proscar
Finasteride tablets are used in men for two main purposes, depending on the product. Which one you take depends on the strength and brand your prescriber chose.
Enlarged prostate (BPH): Proscar and finasteride tablets labeled for BPH treat symptomatic BPH in men with an enlarged prostate. They improve symptoms and reduce the risk of acute urinary retention (suddenly being unable to urinate). They also reduce the risk of needing surgery such as TURP or prostatectomy. With the alpha-blocker doxazosin, they reduce the risk of BPH symptoms getting worse. They are not approved to prevent prostate cancer.
Male pattern hair loss: Propecia and finasteride tablets labeled for hair loss treat male pattern hair loss (androgenetic alopecia) in men only. Efficacy in bitemporal recession (receding at the temples) has not been established. These products are not for use in women.
