Loncastuximab Tesirine-Lpyl falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here's the honest picture with neem and Zynlonta (loncastuximab tesirine). In lab dishes, neem leaf extract slowed down a cellular pump called P-glycoprotein, which helps move certain drugs out of cells and the body. On paper, blocking that pump could let drugs that rely on it build up a bit more than expected. Loncastuximab is listed as one of those P-glycoprotein-linked medicines, so in theory neem might raise its levels and effects.

I want to be clear though: this has only been seen in test-tube and animal studies, never in real people, so it's quite speculative. Still, this is a powerful chemotherapy-style cancer drug with serious side effects (low blood counts, swelling, fatigue, liver changes), so I wouldn't add neem on your own. Please run it by your oncology team or pharmacist first.

  • Loncastuximab tesirine-lpyl injection is used to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not respond to or has returned after treatment with at least two other cancer treatments. Loncastuximab tesirine-lpyl is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
