Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for Lorbrena (lorlatinib). Your body relies on a liver enzyme called CYP3A4 to break this medication down. In lab dish studies, neem leaf extract seems to slow that same enzyme down a bit. If that happened in real life, lorlatinib could clear more slowly and build up in your blood, which can mean stronger effects and more side effects (think things like swelling, weight gain, high cholesterol, mood or memory changes, or nerve tingling).

I want to be honest, though: this is only a theoretical concern from test-tube work, never actually reported in people. Since lorlatinib is a serious cancer drug, I wouldn't add neem without first running it by your oncologist or pharmacist.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
