Lurbinectedin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the basic idea with Zepzelca (lurbinectedin): your liver uses an enzyme called CYP3A4 to clear this chemo drug out of your system. Neem leaf extract has shown, in lab dish studies only, that it can slow that same enzyme down. If that were to happen in a real person, your body might break down the lurbinectedin more slowly, letting it build up to higher levels than intended.

With a chemo drug, more drug in your system can mean worse side effects, things like low blood counts, infections, nausea, fatigue, and liver irritation. That said, this is purely a theoretical concern right now, never actually seen in people. Still, because we're talking about cancer treatment, please loop in your oncology team before using any neem product.

What Is Lurbinectedin Injection Used For?

Brand name: Zepzelca

Zepzelca (lurbinectedin) injection is approved for adults with small cell lung cancer (SCLC) in two settings. First, Zepzelca is used in combination with atezolizumab (or atezolizumab and hyaluronidase-tqjs) as maintenance treatment for extensive-stage small cell lung cancer (ES-SCLC) in patients whose disease did not progress after initial treatment with those same immunotherapy drugs plus carboplatin and etoposide.

Second, Zepzelca is approved as a single agent for adults with metastatic small cell lung cancer whose cancer has worsened during or after platinum-based chemotherapy. This second use was granted accelerated approval based on tumor response rates, and continued approval depends on results from confirmatory clinical trials.
