Here's the simple version: Muraglitazar is a diabetes medicine that lowers your blood sugar, and neem may gently nudge blood sugar down too. When you stack two things that both push sugar lower, you can end up lower than you planned. That raises the chance of hypoglycemia, which can feel like shakiness, sweating, dizziness, a racing heart, hunger, or confusion.

I want to be honest about the evidence, though. The concern here mostly comes from lab and animal work, with only some human research suggesting neem lowers blood sugar. So it's a reasonable caution, not a red alert. If you take neem regularly alongside Muraglitazar, check your blood sugar a bit more closely at first and chat with your pharmacist or doctor before adding or stopping anything.
