Why this applies to Ritonavir
Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with neem and ritonavir. Lab studies (done in test tubes, not people) suggest neem leaf extract can slow down an enzyme called CYP3A4. Ritonavir is itself a strong CYP3A4 blocker and is partly handled by this same enzyme, so in theory neem might nudge ritonavir levels a little higher. That could mean more of ritonavir's usual side effects, like nausea, an upset stomach, diarrhea, or an unpleasant taste.
I want to be honest though: this is theoretical and has never actually been reported in humans, so I wouldn't lose sleep over it. Still, ritonavir is a key part of HIV treatment, so before adding any neem product, please run it by your pharmacist or doctor first.

Ritonavir (brand name Norvir) is used together with other HIV medicines to treat HIV-1 infection in adults and children older than one month of age. It is always taken as part of a combination regimen — never on its own.
Norvir oral powder is specifically indicated for pediatric patients with HIV-1 infection. Norvir tablets and oral solution are indicated for both adults and pediatric patients with HIV-1 infection. Ritonavir tablets (generic) are also indicated for the treatment of HIV-1 infection in combination with other antiretroviral agents.
