Spironolactone falls under ANTIHYPERTENSIVE DRUGS — the group Nicotinamide Riboside interacts with.

Theoretically, nicotinamide riboside may have additive effects with antihypertensive drugs, potentially increasing the risk for hypotension.
One small clinical study suggests that nicotinamide riboside can modestly reduce blood pressure in hypertensive patients. However, other clinical research did not find a reduction in blood pressure when compared with control.

Here's the gist: spironolactone (Aldactone) lowers your blood pressure and helps your body shed extra fluid. There's a thought that nicotinamide riboside might gently nudge blood pressure down too. If that holds true, taking the two together could stack their effects and leave your pressure a little lower than expected.

I want to keep this in perspective, though. This is a theoretical concern based on how things work, not a proven problem. One small study hinted NR lowered blood pressure, while others showed no change. If you do combine them, just keep an eye out for dizziness, lightheadedness when standing up, or feeling unusually tired. Mention to your pharmacist or doctor that you'd like to start NR, and they can help you watch your numbers.

About Spironolactone

What Is Spironolactone Used For?

Brand names: Aldactone, Spiroton, Carospir

Spironolactone tablets (including Aldactone) are used to treat moderate-to-severe heart failure, high blood pressure, and fluid retention (edema) caused by liver cirrhosis or nephrotic syndrome. They are also used to diagnose and manage primary hyperaldosteronism — a condition where the adrenal glands produce too much of the hormone aldosterone.

Spironolactone oral suspension shares several of these uses — heart failure, high blood pressure, and edema from liver cirrhosis — but is not the same as Aldactone and is not therapeutically interchangeable with it. Your prescriber will choose the right form for your situation.
